Description

• Conduct assessments of patients in both inpatient and outpatient environments and tailor treatment plans accordingly
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work in concert with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Guide patients and their families through educational sessions on therapeutic goals, discharge planning, and at-home exercise routines
• Maintain thorough documentation of evaluations, treatments, and patient progress per facility and regulatory frameworks
• Engage in collaborative rounds and interdisciplinary conferences to deliver integrated care

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualification from a recognized program

Certifications

• Valid state licensure as a Physical Therapist
• Preferred CPR certification
• Additional specialized certifications (e.g., in neurology, geriatrics) are advantageous but not required
