We're growing — and we're looking for a skilled, relationship-driven LPN to join our clinical team.Position Overview The Licensed Practical Nurse will serve as a key clinical and operational partner, responsible for client assessments, individualized care plan development, and ongoing oversight of client care. This role requires both clinical expertise and strong communication skills to build lasting relationships with clients, families, and our care team.Key ResponsibilitiesConduct initial nursing assessments for new clients prior to the start of careComplete 30-, 60-, and 90-day reassessments to monitor client status and care effectivenessDevelop, implement, and review individualized care plans in accordance with company policy, state regulations, and physician ordersUpdate and revise care plans as client conditions changeAssist with scheduling coordination to ensure appropriate caregiver-to-client alignmentParticipate in client meet-and-greets to establish rapport and set care expectationsMaintain accurate and timely clinical documentation within the agency's care management platformServe as a liaison between clients, families, physicians, and the care teamSupport agency compliance with South Carolina DHEC home care regulationsQualificationsActive South Carolina LPN license in good standing (required)Minimum 1–2 years of nursing experience; home health or home care experience preferredStrong clinical assessment and care planning skillsTech-savvy — comfortable with electronic health records, care management software, and digital communication toolsExceptional interpersonal and relationship management skillsHighly organized with strong attention to detailReliable transportation and valid SC driver's licenseAble to Pass a Drug screen and Background CheckWhat We OfferFlexible scheduling (part-time and full-time options)Supportive, team-oriented work environmentMeaningful, mission-driven work in your communityOpportunity to grow with an expanding agencyVision and Dental Insurance401K PlanJoin our team and contribute to improving the lives of our clients through compassionate and skilled care coordination!
