What You'll Be Doing Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds. Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities. Manage medications, diagnostics, treatment plans, and follow-up care. Collaborate with physicians, nurses, therapists, caregivers, and facility leadership. Identify barriers to healing and implement evidence-based interventions. Support continuity of care while helping improve clinical outcomes. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred). Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred. Strong clinical judgment and confidence managing medically complex patient populations. Valid driver's license and ability to travel throughout the Twin Cities Metro Area. Some providers manage routine care.
