Registered Nurse (RN) - Case Management

Clinical Coordinator – Behavioral Health

Schedule: 8:00 AM - 5:00 PM (M-F)

Shift Details: Days - 5 x 8

Contract Length: 143 weeks

Guaranteed Hours: TBD

Requirements:

• Valid Registered Nurse (RN) license issued by the appropriate Board of Nursing
• Basic Life Support Certification
• At least two years of experience in a medical office setting

Roles & Responsibilities:

• Provide direct patient care by assessing, planning, implementing, and evaluating nursing interventions to ensure optimal outcomes.
• Collaborate with physicians, fellow nurses, and other healthcare professionals to develop and implement individualized patient care plans.
• Maintain accurate and detailed medical records of patients' conditions, treatments, and progress, reporting any changes or concerns promptly to the appropriate team members.
• Administer medications and treatments as prescribed, ensuring adherence to hospital policies and protocols.
• Educate patients and their families on disease management, treatment options, and preventive measures to promote overall wellness.
• Offer emotional support and advocate for patients' rights, ensuring they receive compassionate and respectful care.

Compliance / Notes:

• Some Saturdays may be required.
• Acknowledgment of maintenance of confidential information is required.
