In-Home Physician Assistant

Signify Health is seeking a Physician Assistant to perform comprehensive in-home evaluations for diverse patient populations. You will obtain histories, conduct physical exams, order and interpret diagnostics, and document findings in our technology platform to support risk adjustment and care planning. Collaborating with interdisciplinary teams, you’ll identify gaps in care, educate patients on conditions and medications, and coordinate follow-up with primary and specialty providers. This role suits a mission-driven clinician who is comfortable working independently in the field, using data and digital tools to improve outcomes in a value-based care environment.
