Inpatient Social Worker

Kaiser Permanente is seeking a Licensed Social Worker (Inpatient/Social Services) to support patients and families in our healthcare & medical services department. In this role, you will complete psychosocial assessments, create and coordinate discharge and care plans, provide brief counseling and crisis intervention, and connect patients with vital community resources. You’ll collaborate closely with an interdisciplinary care team to promote safe transitions of care and reduce readmissions. The ideal candidate brings strong assessment, documentation, and communication skills, plus compassion for diverse populations in a fast‑paced inpatient setting. Master’s degree in Social Work and active state licensure required.
