Home Care Services Coordinator & Liaison

Location: Gastonia, NC Location: Gastonia, NC Job Summary The Home Care Services Coordinator and Liaison is the primary relationship manager between the agency and referral sources. Responsible for managing the day-to‑day operations of the home care office and build relationships with hospitals, physician practices, rehab centers, SNF/ALF, case managers and community organizations. This role is responsible for performing various administrative tasks, in‑home aide recruiting and scheduling, coordinating with caregivers, managing client files, ensuring compliance with all state and federal regulations, generating referrals, promoting agency services, developing strategic relationships, and ensuring a positive client experience that supports census growth and organizational goals. This is a highly interactive position that requires high level of professionalism, capability to drive performance within a group, great computer skills and exceptional communication skills. Key Responsibilities Develop and maintain excellent relationships with healthcare providers, social workers, case managers, ALFs, clients, and staff Assist with coordinating the daily operations of the home care agency Educate prospective referral partners, clients, and families regarding home care services and available programs Generate qualified referrals and meet established admission and census growth goals Conduct and represent the agency at regular networking activities, community outreach events, professional associations Implement territory growth plans to increase referral volume, under the direction of leadership Maintain knowledge of agency services, payer sources, eligibility requirements, and industry regulations Coordinate and maintain client and employee schedules Coordinate with caregivers to ensure they have the necessary information and supplies for each assignment Manage client and employee files and ensure they are up-to-date and accurate Ensure compliance with all state and federal regulations, and professional certification licensure Answer phone calls and respond to emails from clients, caregivers, insurance companies, case managers and prospects Assist with billing and invoicing as needed Participate in on‑call rotation after‑hours, weekend, holiday Monitor market trends and competitor activity and provide feedback to leadership Prepare and submit weekly/monthly activity reports, referrals, business development updates Perform other duties as assigned Qualifications Bachelor's degree in Healthcare Administration/Business/Marketing/Nursing/Social Work or related filed, or equivalent experience Prior experience in a healthcare or home care administration/marketing/business development setting (2 Years Preferred) Experience in home health, home care, hospice, senior living, or healthcare services (Preferred) Established healthcare network within a local market (Preferred) Demonstrated success in business development and achieving growth goals Excellent organizational, communication, and relationship-building skills Ability to work independently and as part of a team Proficiency in Microsoft Office and other computer programs Knowledge of state and federal regulations related to home care Valid Driver's License and reliable transportation Health Vision PTO 401(k) (with Employer Match) Mileage Reimbursement Professional Development Opportunities Physical Demands Local travel within assigned territory Ability to attend meetings, community events, and healthcare facilities Ability tosit, stand, walk and drive for extended periods of time and may need to lift up to 25 pounds Working Conditions The Home Care Services Coordinator & Liaison will work in an office environment and may be required to work evenings and weekends as needed.
