The Team Coordinator is responsible for the patient charts, including establishing and creating the chart at admission and maintaining documentation until the patient is discharged and the chart is closed. As the Team Coordinator, you will facilitate the flow of patient information to all members of the Inter-Disciplinary Team (IDT) in a timely manner.

 

Key Responsibilities:

• Manage incoming and outgoing mail and distribute correspondence to executives and appropriate staff.

• Print and prepare CTIs and other required documentation for IDT meetings.

• Answer and direct incoming phone calls in a professional and courteous manner.

• Coordinate patient intake and communication between branch offices.

• Order, track, and maintain inventory of medical supplies for the branch office.

• Manage drop-ship orders and ensure timely delivery of supplies.

• Maintain adequate stock levels of office and medical supplies.

• Order office supplies and oversee general office organization.

• Assist with maintaining a clean, professional, and well-organized office environment.

• Provide administrative support to hospice operations and clinical teams as needed.

Qualifications:

• Previous hospice or healthcare experience preferred.

• Experience ordering, managing, and tracking medical supplies required.

• Strong organizational and multitasking skills.

• Excellent communication and customer service skills.

• Proficiency in handling administrative and office coordination responsibilities.

• Ability to work collaboratively with multiple departments and office locations.
