Home Care RN Nurse Consultant- Part Time

This position may include but is not limited to coverage of areas in Burnsville, Apple Valley and South St. Paul.

FLSA Status: Hourly, Non-Exempt FTE:.5 PT 20 hours/week Flexible Schedule to meet the needs of the sites assigned; may include weekends as needed Hiring Range: Starts at $40/hour depending on qualifications, experience in the field, skills and education acquired

Your Impact:

• Perform medical patient assessments and assist in developing individualized care plans
• Advise and administer medications, treatments, and therapies, as prescribed
• Monitor patient progress and provide ongoing evaluations and updates to the care team
• Educate patients and unlicensed direct care employees on health management and home care procedures
• Coordinate care with other healthcare professionals and community resources
• Maintain accurate and timely documentation of patient care and progress
• Perform other duties as assigned, contributing to the success of the program and the overall mission of the organization

What You'll Need:

• Valid RN license in Minnesota
• 2 years of nursing experience or equivalent, preferably in home health or community health settings.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Department of Human Services background clearance
• Valid driver's license, reliable vehicle, acceptable driving record
• Ability to effectively communicate in English, both written and verbally
