Overview This pharmacist role applies knowledge and skills to deliver safe, efficient, and cost-effective pharmaceutical care within the scope of practice. Responsible for patient care and developing professional relationships within the multidisciplinary team. Schedule and Location Location: Intermountain Health Heber Valley Hospital, Heber City, Utah. Scheduled weekly hours: 40. Hours and on-call expectations include an 8 on / 6 off schedule with PTO accrual and weekend on-call responsibilities. Schedule rotates to alternate major holidays between the two 8 on / 6 off pharmacists. Hours: Mon-Sun 07:30 to 18:00; Mon 07:00 to 17:30; On-call: Friday 18:01–07:29; Saturday 18:01–07:29; Sunday 18:01–06:59. Essential Functions Review patients’ medication orders for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug–drug interactions. Make evidence-based interventions to optimize medication use and ensure safe, appropriate, and cost-effective therapies; accurately dispense medications. Oversee and/or participate in ensuring medications are prepared and delivered timely to care areas and stored securely for stability and workflow. Provide counseling to patients and promote quality medication education and adherence; ensure patients are informed and receive professional service. Complete regulatory and compliance training; maintain clear documentation of regulatory requirements. Remain knowledgeable and compliant with state and federal laws, pharmacy policies, procedures, and SOPs. Supervise and direct activities of interns, technicians, and support personnel as required. Provide medication-related clinical consultation and clarification to other health professionals. Identify and solve problems (equipment, maintenance, drug availability, staffing, patient care concerns) and identify opportunities for process improvements to enhance patient satisfaction and safety. Participate in departmental meetings; support clinic/hospital and system directives, including clinical, compliance, operations, standardization, and customer service. Support local and enterprise pharmacy leadership initiatives. Qualifications Minimum Doctor of Pharmacy or Bachelor of Science in Pharmacy from an accredited institution (education verified). Current license to practice pharmacy in the state; ability to dispense controlled substances. Current knowledge of state and federal regulations governing pharmacy practice in the state of practice. Effective communication, people management, and project management skills. Immunization certification if applicable to practice site needs. Preferred Experience in an integrated healthcare delivery system; Board Certification; Hospital or Health System Residency. Physical Requirements Frequent reading of information, labels, documents, and monitors; ability to assess customer needs quickly and accurately.
