Job Title: ICU Registered Nurse Profession: RN Specialty: ICU Duration: 13 weeks Shift: 12hr Night Shift Hours per Shift: 36 Experience: 18 months to 2 years of ICU RN experience License: Current RN License Certifications: BLS, ACLS, NIHSS Must-Have: - Minimum 18 months to 2 years of ICU RN experience - BLS, ACLS, NIHSS certifications Description: The facility has 109 licensed beds with no trauma designation. It is not a teaching hospital and does not have residents or students. The unit includes 10 ICU beds and 4 PCU beds, with the PCU utilized mainly in surge capacity. The average census is approximately 7.3 patients per day. Patient types and common diagnoses include sepsis, COPD, CHF, chest pain, MI, NSTEMI, stroke, TIA, new onset cardiac arrhythmia, uncontrolled DM and DKA, minor trauma, alcohol withdrawal, ARDS, and respiratory illnesses such as flu, COVID, and pneumonia. Patient ratios are typically 1:2-3, depending on acuity, and RNs may often have 3 patients in their assignment. There is no CNA available overnight, with a 1:10 ratio only during the day shift. Skills required include: - Stroke experience and NIH assessments - Cardiac rhythm interpretation - Management of arterial lines and central venous pressures - Experience with critical care drips - Chest tube management - Peripheral intravenous starts - TNK administration - Ventilator management supervised by respiratory therapy - Tracheostomy care - Nasogastric tube placement - Drain and tube management including Dobhoff, PEG tubes, Hemovac, JP drains, and foley management - PCA pump management - Wound care - Ability to complete 12-lead EKGs on demand Medication management includes titration of critical care drips, cardiac drips, heparin, blood products, and intravenous fluids. Support within the department includes a house supervisor and a charge nurse on duty. Phlebotomy services are available for any necessary procedures. The IV/VAT team is only available during the day shift. A hospitalist is on duty 24/7, and if intubation is required, the hospitalist will be first in line, followed by a CRNA and then the emergency department if needed. There is one intensivist who rounds Monday to Friday around 6 AM, with night RNs providing reports to him. In urgent situations, there is a contact for overnight issues. Bedside and central telemetry monitoring RNs will oversee telemetry for the unit. Technology and equipment used include EPIC for electronic medical records, Alaris for IV pumps, Omnicell for medication dispensing, and Phillips for monitors. Floating may occur within the facility to Med/Tele, PCU, or ED holds as a task RN. Regional floating may also be required within the scope of practice. Weekend rotations are expected at a minimum of every other weekend. Travelers will work 75% of holidays, with requests for time off needing manager approval. Shift times require being clocked in before 1845 but not before 1838. Schedules are on a 6-week cycle. Unit culture emphasizes teamwork and open communication, with an openness to constructive feedback.
