Social Worker

Licensed Independent Social Worker (LISW) We are seeking a compassionate and experienced Licensed Independent Social Worker (LISW) to support the mental and emotional well-being of individuals and families in a rural healthcare setting. The LISW will provide clinical assessments, individualized treatment planning, and evidence-based behavioral health interventions while working closely with an integrated healthcare team. Job Responsibilities: Conduct comprehensive psychosocial assessments and identify behavioral health needs. Develop and implement individualized treatment plans. Provide evidence-based therapy and behavioral health interventions. Support patients experiencing depression, anxiety, trauma, substance use, and adjustment disorders. Collaborate with physicians, nurses, and behavioral health professionals. Provide crisis intervention, counseling, and appropriate referrals. Maintain accurate and timely clinical documentation. Promote whole-person care and advocate for patients and families. Support underserved and rural populations with compassionate, patient-centered care. Requirements: Master’s Degree in Social Work from an accredited program. Minimum 2 years of clinical experience preferred . Rural or medically underserved healthcare experience preferred.
