Compassionate Hospice RN Case Manager

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead care at patients' homes or wherever they call home. You will assess, plan, and deliver skilled nursing, coordinating with an interdisciplinary team to ensure compassionate end-of-life care. The role emphasizes education for patients and families, symptom management, and timely documentation. A current RN license and hospice experience are preferred; strong communication and empathy are essential. #J-18808-Ljbffr
