Work Environment\n

This position is fully remote, allowing for a flexible work life while providing support to families in the Union City, Georgia area and beyond. Our Hospice Center operates with a holistic approach, emphasizing emotional and psychosocial well-being for each child and family. Team collaboration happens virtually, maintaining strong communication and connection among team members to meet individual family needs.

Key Responsibilities\n

• Conduct thorough evaluations and assessments that identify the unique needs of each child and their family.
\n
• Engage collaboratively with families to create and implement Individualized Family Service Plans (IFSP), ensuring that all strategies are tailored to the child's developmental status and family priorities.
\n
• Regularly monitor and assess outcomes as part of a multidisciplinary team, contributing to continuous improvement in care strategies.
\n
• Provide evidence-based support, training, and resources to families, enhancing their understanding of their child’s developmental milestones and fostering their confidence in nurturing those skills.
\n
• Utilize a coaching model and Natural Learning Environment Practices to facilitate learning through everyday routines.
\n
• Ensure compliance with all relevant Federal and State regulations, maintaining the highest standards of care and ethical practice.
\n
• Foster a trusting and supportive environment to encourage open communication with children, families, and team members, which is essential for their success.
\n

Patient Population / Client Focus\n

Our primary focus is on children, particularly those aged birth to three years, including infants and toddlers with disabilities or developmental delays. You will work closely with families, providing tailored early intervention services that address physical, cognitive, social/emotional, and communication development. Your role will be critical in helping families navigate the systems of care and ensuring they have access to necessary support.

Qualifications\n

• A Master's Degree or Doctorate in Social Work is required.
\n
• Current licensure as a Licensed Clinical Social Worker (LCSW) issued by the Department of Public Health is essential.
\n
• Possession of a valid driver’s license to facilitate travel for any in-home visits as needed.

• Strong capability to adapt therapeutic approaches to meet the diverse learning styles of children and their families.

• Excellent organizational and time management skills to handle a variety of cases efficiently and effectively.

• Ability to communicate effectively and respectfully, fostering collaborative relations with team members.

Preferred Experience\n

• Minimum of one year of experience in early intervention services working specifically with children and their families.

• Demonstrated commitment to Natural Learning Environment Practices and experience using a coaching model in practice.

Compensation and Benefits\n
