A leading healthcare facility is seeking a skilled Diabetes Pharmacist to join their team on a contract basis in Lawton, OK, with some float required to Carnegie and Anadarko. This role offers the opportunity to directly impact patient care by practicing intensive diabetes management within a collaborative practice setting. The assignment is slated to begin as soon as possible and will last for one year, with potential for four one-year extensions.
Schedule:
- 8-hour day shifts
Key Responsibilities:
- Process and dispense daily prescription orders from healthcare providers, including new prescriptions and refills
- Provide thorough medication counseling to patients to ensure safe and effective use
- Oversee pharmacy support personnel to maintain efficient workflow
- Deliver intensive diabetes management based on a comprehensive pharmacy-based collaborative practice agreement
- Utilize current evidence-based guidelines, such as the American Diabetes Association Standards of Care, to inform clinical decisions
- Demonstrate proficiency in the placement and data interpretation of continuous glucose monitors (CGMs) to make appropriate medication adjustments
Qualifications:
- Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD)
- Valid state pharmacy license
- Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) from the Board of Pharmacy Specialties, or completion of a Post Graduate Year One (PGY1) residency
