Job Summary:
We are seeking a highly motivated and skilled Physical Therapist (PT) to join our team on a Per Diem basis. As a Physical Therapist (PT) at Desert Peak Care Center, you will be responsible for delivering high-quality patient care, assessing patients' conditions, developing and implementing treatment plans, and collaborating with other healthcare professionals to achieve optimal patient outcomes.

Responsibilities:

• Assess patients' conditions and develop individualized treatment plans to achieve optimal patient outcomes

• Conduct physical therapy sessions, including exercises, stretches, and other modalities to improve patients' strength, flexibility, and mobility

• Monitor patients' progress, adjust treatment plans as needed, and communicate with patients and families to ensure seamless care

• Collaborate with other healthcare professionals, including physicians, occupational therapists, and speech-language pathologists, to provide comprehensive patient care

• Maintain accurate and detailed records of patient care, treatment plans, and progress

• Stay up-to-date with best practices in physical therapy and adhere to professional standards and guidelines

Requirements:

• Bachelor's or Master's degree in Physical Therapy (PT) from an accredited program

• Current Physical Therapy license in the state of Arizona

• Excellent communication, interpersonal, and problem-solving skills

• Ability to work effectively in a fast-paced environment and adapt to changing priorities
