Description

• Assess patients in either an inpatient or outpatient environment and create personalized treatment plans
• Deliver physical therapy interventions aimed at mobility, strength, balance, and functional recovery
• Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Guide patients and families on therapeutic objectives, discharge processes, and home exercise routines
• Document evaluations, interventions, and progress following facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to guarantee coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent recognized degree

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
