Home Health Education Coordinator

Well Care, LLC, a WellCare Health Plans company, is seeking a Home Health Education Coordinator to design and deliver impactful education for members receiving care at home. In this role, you will develop culturally appropriate materials and programs that support Medicaid and Medicare members, families, and caregivers in managing chronic conditions and following care plans. You will partner closely with nurses, social workers, and care managers to identify education needs, support regulatory compliance, and improve outcomes. Responsibilities include training field staff, documenting education encounters, evaluating program effectiveness, and supporting quality improvement initiatives in a mission-driven, data-informed environment focused on vulnerable populations.
