Compensation Salary Range: $115,000 $125,000 per year Location: Bronx, NY (Montefiore) Onsite / Field-Based Position Summary As a Nights Hospice Field Case Manager (RN), you will provide compassionate, patient-centered care to hospice and palliative patients across community and facility settings. This role involves performing comprehensive physical and psychosocial assessments, developing individualized care plans, and supporting patients and families through end-of-life care. You will collaborate with interdisciplinary teams to ensure high-quality, culturally sensitive care while promoting comfort, dignity, and quality of life.

Responsibilities

• Conduct physical and psychosocial assessments during hospice and palliative care visits
• Develop and implement individualized nursing plans of care based on patient needs
• Perform visits in home, community, and facility settings with electronic documentation
• Coordinate with physicians to obtain and update care orders as needed
• Supervise, train, and evaluate home health aides and homemakers
• Participate in on-call coverage and weekend rotation as required
• Ensure delivery of culturally sensitive, compassionate end-of-life care

Requirements

• Graduate from an accredited School of Nursing
• Active New York State Registered Nurse (RN) license; valid NYS Driver's license

Skills

• Strong clinical assessment and hospice care management skills
• Ability to work independently in a field-based environment
• Effective communication and interdisciplinary collaboration skills
• Proficiency in electronic documentation and basic computer applications (Word, Excel)
