Summary:

The Early On Service Provider is responsible for assessing the developmental needs of children, birth to age three, and coordinating services to support each child and family. The provider develops and implements Individualized Family Service Plans (IFSPs), coaches caregivers through embedded learning opportunities, and provides families with information related to child development to support outcomes in the child's natural environment.


Duties and Responsibilities (may include, but not limited to):

• Implement and monitor Individualized Family Service Plans (IFSPs) in accordance with Part C requirements.
• Collaborate with the Early Childhood Programs & Services team to review referrals and assess family and child needs.
• Conduct intake meetings with referred families, establish rapport, develop IFSP outcomes, and schedule follow-up services.
• Provide services primarily in the home and other natural environments, coaching caregivers on strategies that support development through daily routines, play, and communication.
• Administer appropriate developmental assessments; score and interpret results; and share findings with families and the Early On team.
• Writes progress reports, Individual Family Service Plans (IFSP), reviews, parent contact forms, and transition plans on assigned children. Communicates the child's progress to the family.
• Complete required documentation including IFSPs, progress reports, service notes, parent contact logs, and transition plans in a timely and compliant manner.
• Communicate regularly with families regarding child progress and support family-centered goal setting.
• Maintain accurate and up-to-date records using district technology systems.
• Collaborate with families, school personnel, and community agencies to support child and family outcomes.
• Participate in Tot Spot instructional playgroups and Developmental Assessment Clinics throughout the year.
• Participate in weekly Early On team meetings (Birth to Three).
• Perform other related duties as assigned.

While performing the duties of this job, the Early On Service Provider must be able to do the following:

• Lift up to 50 pounds.
• Frequently stand, walk, and move throughout the workday.
• Occasionally reach, stoop, bend, kneel, or crouch.
• Use hands for grasping, pushing, and fine motor tasks.
• Maintain visual abilities including close vision, color vision, depth perception, and focus adjustment.
• Drive to and from service locations within the assigned geographic area.
• Perform repetitive hand and wrist movements for computer and office tasks.

Qualifications:

Education:

• Meets State of Michigan personnel requirements for discipline or profession required.
• Michigan Association for Infant Mental Health Endorsement® (IMH-E®) preferred.
• High school diploma required.

Experience:

• Experience working with children birth to age three required.
• Experience conducting developmental assessments, providing home-based services, and collaborating with community agencies preferred.

Skills/Other:

• Ability to work collaboratively as part of a multidisciplinary team.
• Strong communication skills with families and professionals.
• Ability to complete accurate documentation and meet state and federal timelines.
• Proficiency in using technology for documentation and communication.
• Ability to organize workload and manage time effectively.

• Ability to work a flexible schedule as needed.
• Commitment to family-centered practices and culturally responsive service delivery.

Salary Range: The salary will be commensurate with education and demonstrated experience of the successful candidate to fulfill the requirements of the position as determined by administration.
