We are seeking a part-time skilled Nurse Practitioner or Physician Assistant to join our Primary Care Office, providing comprehensive patient care through a combination of office visits and house calls. This role offers flexible hours and the opportunity to deliver personalized, high-quality care in diverse settings.
Responsibilities
- Provide patient care through office visits and house calls
- Conduct clinical assessments and develop treatment plans
- Deliver patient education to support health management
- Document medical records accurately and timely
- Manage medication therapies
- Coordinate follow-up care to ensure continuity
- Collaborate with healthcare team members
- Adapt to flexible scheduling needs
