Physical Therapist — Key Responsibilities Conduct thorough patient evaluations: Assess mobility, functional limitations, safety risks, and therapy needs to build a clear roadmap for in-home rehabilitation. Create personalized treatment plans: Design evidence-based, goal-driven plans tailored to each patient's condition, driving meaningful gains in strength, independence, and confidence. Deliver skilled, hands-on care: Provide one-on-one interventions that improve mobility, balance and function — directly shaping outcomes and quality of life. Educate and empower patients and families: Teach home exercise programs, safety techniques, and care strategies that sustain progress between visits and prevent setbacks.
