Physical Therapist
Heritage Grove Estates is seeking a Full-Time or PRN Physical Therapist to join our in-house rehabilitation team in Topeka. We’re looking for a clinician who is passionate about patient-centered care and enjoys working in a collaborative, team-driven environment. New graduates are encouraged to apply!

$5,000 Sign-On Bonus Available

Key Responsibilities
• Provide physical therapy services to improve residents’ mobility, strength, balance, and overall functional independence
• Develop, implement, and adjust individualized treatment plans based on physician orders and resident progress
• Perform comprehensive evaluations to assess physical function, range of motion, strength, and mobility
• Utilize appropriate therapeutic techniques, exercises, and modalities to achieve optimal outcomes
• Educate residents, families, and caregivers on therapy goals, home exercise programs, and safety strategies
• Maintain accurate, timely documentation in compliance with company policies and state regulations
• Communicate resident progress, changes, and clinical observations with the interdisciplinary team
• Participate in care planning meetings and collaborate with nursing, therapy, and medical staff

Qualifications
• Active Kansas Physical Therapist (PT) license
• In good standing with all applicable licensing boards
• Experience in skilled nursing or long-term care settings preferred
• Familiarity with Net Health documentation system is a plus
• Strong communication and interpersonal skills
