Travel Registered Nurse Home Health Job

Overview

Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings and share with the interdisciplinary team.
- Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity of care; update as patient needs change.
- Gather data on physical, psychological, social, spiritual, and cultural factors influencing health status and integrate into the Plan of Care.
- Maintain up-to-date patient records, documenting visits, contacts, problems, plans, actions, and goals; ensure changes are reflected promptly.
- Provide care with appropriate infection control measures; educate patients and families on care, medications, home safety, and self-care practices.
- Supervise and provide clinical directions to Home Health Aides, STNAs, and LVNs as assigned to ensure continuity and quality of service.

Requirements
- Current state RN license — Required
- CPR certification — Required
- CHPN certification — Preferred
- Valid driver’s license — Required
- Reliable transportation and proof of auto insurance — Required
- 1-2 years RN experience in a clinical care setting — Required
- 2+ years of home health experience — Required
- OASIS experience — Required
- Homecare Homebase software experience — Required
- Education: Associate Degree in Nursing or Nursing Diploma — Required; Bachelor of Science in Nursing — Preferred
