Jail Transitions Case Manager

SUMMARY
Responsible for facilitating the successful reintegration of individuals exiting the jail system into the community. This role provides support, resources, and guidance to clients, helping them navigate challenges related to housing, employment, mental health, and substance use. The Jail Transition Case Manager will work collaboratively with various stakeholders, including law enforcement, social services, and community organizations, to promote a smooth transition and reduce recidivism. Description Responsible for facilitating the successful reintegration of individuals exiting the jail system into the community. This role provides support, resources, and guidance to clients, helping them navigate challenges related to housing, employment, mental health, and substance use. The Jail Transition Case Manager will work collaboratively with various stakeholders, including law enforcement, social services, and community organizations, to promote a smooth transition and reduce recidivism. Position Full-time Pay $27.65 - $29.70/ hour Benefits Generous Paid Time Off -- New Employees accrue up to 31 paid days off, including 15 flexible use PTO days, 10 paid holidays, 2 personal days, as well as the final week of the year. Maintains a reliable attendance record and adheres to scheduling requirements based on business and clients’ needs. Conducts comprehensive assessments of clients’ needs, including risk factors, strengths, and areas requiring support. Develops, implements, and updates personalized transition plans that address housing, employment, education, and health care needs. Connects clients with community resources such as housing assistance, job training programs, mental health services, and substance abuse treatment, and assists in obtaining necessary documentation for applicable benefits. Maintains ongoing case management with regular follow-ups and progress monitoring, adjusting transition plans as necessary. Provide appropriate crisis intervention and support to clients facing challenges during their transition period. Advocates on behalf of clients to ensure they receive necessary services and support from various agencies and community organizations. Facilitates groups or meetings with clients or partners on life skills, job readiness, and personal development. Understands and practices safekeeping of client-protected health information per Bridgeways’ HIPAA policies and procedures, utilizing Bridgeways’ Electronic Medical Record system in a timely and accurate manner to ensure compliance. Represents Bridgeways in community and regional activities, committees, and work groups as requested. EDUCATION And/or EXPERIENCE Bachelor’s degree in social work, psychology, criminal justice, or a related field (Master’s degree preferred) or equivalent experience. Minimum two years’ experience working in a case management, social services, or criminal justice (criminal justice system preferred). Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.REASONING ABILITY
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to understand broad conceptual terms and specify fine details of concepts used in developing and implementing program, policies, and procedures.CERTIFICATES, LICENSES, REGISTRATIONS
Valid Washington State Driver’s License. Requirement to pass Background check in accordance with Customer Guidelines and Requirements. Must be authorized to work in the United States. Certified Case Manager, Mental Health First Aid, Peer Counselor certification are a plus. Other Skills And Abilities Strong interpersonal and communication skills with ability to build rapport and trust with diverse populations. Knowledge of community resources and services related to reentry. Ability to learn and utilize electronic medical records for confidential and safe record-keeping that conforms to agency requirements. Ability in problem-solving and crisis management.PHYSICAL DEMANDS
While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to walk and climb stairs or balance. The employee is occasionally required to stand, stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.WORK ENVIRONMENT
The employee works in an office space provided in either an administrative office environment or clinical facility environment. The noise level in either work environments is usually moderate. The employee also performs work in the community, to other social service and governmental agencies, and to locations where individuals experiencing homelessness congregate and live, including locations that are not suitable for human habitation.
