About your role Are you a Qualified Professional who wants to be a part of the NC Medicaid transformation and serve our clients in need? Our Care Managers are Qualified Professionals who deliver tailored care management services to clients with complex behavioral health and healthcare needs such as mental illness, substance use disorders, intellectual and developmental disabilities, and/or traumatic brain injuries. Support for these clients includes housing, employment, food insecurity, physical health, finances and safety which may be affecting their well‑being. In this role, you will lead a team of Care Manager Extenders through service provision to meet or exceed the established key performance indicators and desired client outcomes. This role includes travel up to 50% or more of the time, to serve our clients where they are in their journey. Perks of this role Pay starting at $45,000 annually Required qualifications Bachelor’s Degree in a related field (health, psychology, sociology, social work, nursing, or another relevant human services area) and at least 2 years of experience working with the MH/SU/IDD population Bachelor’s Degree in an unrelated field (health, psychology, sociology, social work, nursing, or another relevant human services area) and at least 4 years of experience working with the MH/SU/IDD population Master’s Degree in a related field (health, psychology, sociology, social work, nursing, or another relevant human services area) and at least 1 year of experience working with the MH/SU/IDD population Full or provisional licensure or certification with the appropriate North Carolina board (LCSW, LCMHC, LCAS, LPA, LMFT, etc.) Registered Nurse (RN) in North Carolina and at least 4 years of experience working with the MH/SU/IDD population Benefits Full Time Employees: Paid vacation days that increase with tenure Separate sick leave that rolls over each year Up to 10 paid holidays* Medical, dental, vision benefit plan options Training, development and continuing education credits for licensure requirements All Employees: 401K Free licensure supervision Pet insurance Employee assistance program Perks at Clarvida – national discounts on shopping, travel, Verizon, and entertainment Mileage reimbursement Cellphone stipend Equal Opportunity Employer Clarvida is an equal opportunity employer with a commitment to diversity.
