The Certified Nurse Aide (CNA) provides compassionate, person-centered care that promotes each resident's dignity, independence, safety, comfort, and overall well-being.  Working as an integral member of the Resident Care team, the CNA delivers hands-on care and clinical support to Memory Care residents within the scope of their state certification and community policies. 

The CNA assists residents with activities of daily living; obtains and documents resident observations and clinical information within the scope of practice; recognizes and promptly communicates changes in residents' physical, cognitive, emotional, or behavioral condition; and contributes to the development of a safe, supportive, and engaging environment.  Through attentive care, careful observation, and meaningful relationships with residents, the CNA helps promote each resident's highest level of independence and quality of life.

QUALIFICATIONS AND REQUIREMENTS:

  • High School Diploma or GED preferred 
  • Current CNA license/certification required, and certification must be maintained in good standing throughout employment.
  • Minimum one year in long-term care, assisted living, memory care, or healthcare experience preferred 
  • CPR certification (or ability to obtain within 30 days) 
  • Ability to complete all required state caregiver training 
  • Successful completion of dementia care training 
  • Ability to communicate effectively verbally and in writing 
  • Ability to use electronic documentation systems 
  • Ability to work flexible schedules, including weekends and holidays 
  • Demonstrate compassion, patience, professionalism, and emotional maturity 
  • Excellent interpersonal and customer service skills 
  • Reliable attendance and punctuality 

ESSENTIAL FUNCTIONS:

Responsibilities may include, but are not limited to the following:

 

  • Provide compassionate, skilled, person-centered care to residents in accordance with individualized service plans, community policies, and the scope of practice for a Certified Nurse Aide.
  • Obtain, measure, and accurately document resident vital signs, weights, and other assigned clinical observations, reporting abnormal findings or significant changes promptly to the Resident Care Director, licensed nurse, or supervisor. 
  • Perform systematic resident observations throughout the shift to identify subtle changes in physical, cognitive, emotional, and functional condition. Escalate clinical concerns appropriately. 
  • Accurately document clinical observations, care provided, resident responses, observations, and clinical information in the electronic health record or other approved documentation systems in a timely manner. 
  • Assist in implementing individualized care plans by providing consistent care, documenting resident responses, and communicating observations that support ongoing evaluation of resident needs. 
  • Promote residents' highest level of independence by encouraging participation in activities of daily living while providing appropriate assistance, cueing, supervision, or hands-on support based on each resident's abilities. 
  • Observe residents for signs of illness, injury, infection, dehydration, pain, or other changes in condition, taking appropriate action to ensure resident safety and timely clinical follow-up. 
  • Monitor skin condition during routine care, promptly reporting redness, bruising, skin tears, pressure injuries, or other concerns that may require clinical intervention. 
  • Support fall prevention strategies by maintaining a safe environment, utilizing appropriate transfer techniques and equipment, recognizing resident-specific risk factors, and immediately reporting falls, near misses, or safety concerns. 
  • Respond appropriately to resident emergencies by remaining calm, initiating emergency response procedures, providing CPR or first aid as certified, and promptly notifying the appropriate clinical personnel.
  • Follows written instructions and requests assistance and further instructions with new or unfamiliar situations.
  • Demonstrate professionalism, accountability, and attention to detail while consistently providing safe, compassionate, high-quality resident care.
  • Work collaboratively with Nursing, Housekeeping, Maintenance and Activities.
  • Completes all required and mandatory monthly training related to dementia care, Alzheimer’s disease, and memory-support best practices within established timeframes.
  • Promptly documents and reports behavioral changes or concerns accurately and objectively, following established procedures.
  • Serve as a resident advocate by promoting dignity, honoring resident choice, protecting resident rights, and supporting person-centered decision-making in every interaction.
  • Attend bi-weekly in-service meetings and mandatory meetings
  • Adheres to all community and company policies and performs any other duties as assigned

 

Memory Care Responsibilities

  • Understands the principles of person-centered dementia care and applies them consistently in daily interactions.
  • Builds trusting, therapeutic relationships with residents through patience, empathy, consistency, and meaningful daily interactions. Recognizing that behaviors are often a form of communication rather than intentional actions. Avoid confrontation, arguing, or correcting residents when it may increase anxiety or distress. 
  • Deliver compassionate, person-centered care that recognizes each resident's unique life history, abilities, preferences, routines, culture, and personal choices while promoting dignity, independence, and quality of life.
  • Use redirection, distraction, reminiscence, validation therapy, and other person-centered interventions to reduce agitation, anxiety, confusion, or emotional distress while maintaining each resident's dignity and emotional well-being. 
  • Support individualized daily routines that provide structure, familiarity, consistency, and comfort while minimizing environmental stressors that may contribute to behavioral expressions.
  • Engage residents in meaningful conversation, purposeful activities, sensory experiences, music, reminiscence, exercise, and other individualized programming that supports cognitive, emotional, physical, and social well-being.
  • Assist residents experiencing memory loss during meals, activities, personal care, and social interactions by providing encouragement, cueing, and compassionate support that preserves dignity and promotes success.
  • Create and maintain a calm, safe, secure, and welcoming environment by identifying potential safety risks, reducing overstimulation, promoting orientation cues, and assisting with fall prevention and resident supervision.
  • Observe for potential signs of pain, illness, dehydration, infection, medication side effects, or unmet physical or emotional needs that may contribute to changes in behavior, and report concerns promptly to the nurse in charge.
  • Promote positive family partnerships by treating family members with empathy, professionalism, and respect, recognizing the emotional impact of dementia on loved ones and appropriately communicating resident concerns through established channels.
  • Support the community's dementia care philosophy by participating in ongoing education, applying current best practices, and collaborating with interdisciplinary team members to enhance resident outcomes and quality of life. 
  • REQUIRED COMPETENCIES 

    • Establish and maintain constructive working relationships with co-workers, residents, families and visitors by creating exceptional first and lasting impressions. 
    • Demonstrates compassion, patience, empathy, and emotional resilience when caring for residents living with dementia.
    • Maintains composure and professionalism during challenging resident behaviors or emotionally charged situations.
    • Properly use, remove, and dispose of Personal Protective Equipment (PPE), including gloves, gowns, masks, and eye protection, as required by resident care needs and infection control guidelines.
    • Anticipate resident needs and provide prompt service recovery.
    • Maintain a clean, organized, and sanitary resident environment by following established cleaning, disinfecting, and equipment handling procedures. 
    • Safely handle, transport, and dispose of soiled linens, biohazardous materials, and regulated medical waste in accordance with established policies. 
    • Clean and disinfect resident care equipment and high-touch surfaces between uses, following community procedures and manufacturer recommendations.
    • Maintain confidentiality of verbal and written information pertaining to residents, associates, and community operations
    • Consistently performs all duties in accordance with the service and performance standards established by Parc Communities’ Gold Standards. 
    • Neat appearance, adherence to dress code and good personal hygiene are expected
    • Ability and desire to work as a team member and provide services to residents

    WORK ENVIRONMENT AND PHYSICAL DEMANDS/TRAVEL REQUIREMENTS 

    • Lifting, pushing, or pulling up to 50 pounds, using proper body mechanics
    • Possible exposure to unpleasant odors and chemicals as identified in the MSDS Manual
    • Possible exposure to verbal aggression from vendors, guests, residents and/or family members. 
    • Continuous exposure to clients and/or family members who may be under stress 
    • While performing the duties of this job, the employee is regularly required to stand, bend, talk and hear. The employee is frequently required to walk. The employee is frequently required to use their hands and fingers to touch, handle and feel and required to reach with their hands and arms. Must be able to meet certain physical demands of standing, bending, lifting, stooping or performing other work requiring physical exertion, such as lifting (up to 50 lbs). Must be able to climb flights of stairs.
    • Must be able to sit, stand and walk for extended periods of time.
    • Must be able to endure the resident’s environment, which may be unsanitary and uncomfortable.
    • Must be able to assist residents to roll in bed, sit up from a lying position, stand from a sitting position, transfer assist to and from w/c and provide stand-by assist during resident ambulation or other activities of daily living.
