Nurse Practitioner - Primary Care

Role: Nurse Practitioner providing home-based primary care for medically complex, home-limited adults. Responsibilities Perform in-home and telehealth visits to assess patient needs. Manage chronic and acute conditions through comprehensive evaluations. Coordinate care with an interdisciplinary team to ensure optimal patient outcomes. Order and interpret diagnostic tests and prescribe medications accordingly. Educate patients and caregivers to promote health and prevent hospitalizations. Document all clinical interactions in the EHR and support quality improvement initiatives. Contribute to expanding access to compassionate primary care for vulnerable populations. Qualifications & Skills Current Nurse Practitioner licensure and relevant clinical experience. Strong assessment and clinical decision-making skills. Ability to manage multiple conditions in a home-based setting. Excellent communication and patient education abilities. Comfort with using telehealth platforms and electronic health records.
