Job Description:

- Join a dynamic team as a Case Manager RN working in a Short Term Acute Care facility!
- Enjoy a Day Shift schedule: 5 days, 8-hour shifts (08:00 - 16:00).
- Opportunity to float between various units, enhancing your skills and experience.
- Weekend work required, varying by need and census.
- Engage in a patient-focused role with expected ratios of 1:16.
- Previous experience in case management is essential for success.

Facility Highlights:

- Total Staff Beds: 108
- Scrub Color: Business Casual; scrubs provided for specific departments.
- Charting System: Epic - experience required.

Daily Responsibilities:

- Assist patients and families in developing and fulfilling safe discharge plans.
- Coordinate with community partners for post-acute placements (e.g., Skilled Nursing Facilities, home health, and hospice).
- Work with Case Management Assistants to streamline tasks like DME dispensing and transport arrangements.
- Collaborate with interdisciplinary teams, including physical therapy, social services, and rapid response teams.

Required Qualifications:

- Experience: At least 1.5 years in case management.
- Certifications: BLS required.
- Previous Charge Experience: 1.5 years preferred.
- Knowledge: Understanding of CMS standards, HIPAA, and medical necessity criteria.
