How you’ll make an impact in this role

• Serve as a central liaison connecting management, medical staff, health plans and community organizations to ensure clear communication and collaborative care.

• Coordinate patient treatment plans while actively promoting clinical quality initiatives and staff educational efforts.

• Track key clinical outcomes and evaluate care data to recommend meaningful process and operational improvements.

• Advocate for vulnerable populations by connecting uninsured and underinsured patients with essential follow-up care, financial support and community resources.

What minimum requirements you’ll need

Licensure / Certification / Registration

• Required Credential(s):

• Licensed Compact State Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.

• Preferred Credential(s):

• BLS Provider. American Heart Association or American Red Cross accepted.

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
