Support patients and families in understanding health needs and next steps. Document care accurately using our EHR system. Participate in care huddles and team-based quality improvement initiatives. Serve as a key member of our Patient-Centered Health Home model.

Required: CNA certification, active and in good standing with the State of North Carolina. HS Diploma/GED. Current CPR/BLS certification. Valid Driver's License.

Preferred: One (1) year of clinical experience in a primary care, community health, or outpatient setting. Experience with underserved or rural populations. Familiarity with team-based care or PCMH/PCHH models. Bilingual.
