Role: Advanced Practice Clinician - Nurse Practitioner / Physician Assistant
Program: Senior Community Care (SCC)


Hours: 40/week


Position Overview

The SCC program provides integrated, longitudinal care for high-risk members in facility settings. The APC works within an interdisciplinary team to deliver comprehensive assessments, manage acute and chronic conditions, and support advanced illness planning. This role offers strong work-life balance with flexible scheduling, no on-call, no weekends, and no holidays.

Schedule & Logistics

• Shift: 8 AM - 5 PM
• Days: Monday-Friday
• Dress Code: Medical attire

Primary Responsibilities

• Comprehensive Assessments
• Management of Acute & Chronic Conditions
• ICD-10 Documentation
• Interdisciplinary Collaboration
• Advanced Illness & Care Planning
• Patient/Caregiver Education

Required Qualifications

• Active, unrestricted state license
• NP: Accredited MSN or DNP
• PA: Accredited PA program
• National certification (AANP/ANCC or NCCPA)
• Active DEA
• Ability to meet physical job requirements
• Reliable transportation
• Collaborative practice agreement (if applicable)
• Full COVID-19 vaccination
• Drug-free workplace compliance

Preferred Qualifications

• 1+ year clinical experience (long-term care preferred)
• Experience with complex behavioral, social, or functional needs
• Knowledge of geriatrics, chronic illness, advanced illness
• EMR proficiency

• Area: Orangeburg, SC
• During Shift - 2 facilities
