- Assess Resident functional abilities and develop individualized OT treatment plans targeting activities of daily living, mobility, safety, and independence.
- Implement, monitor, and adjust therapy interventions; document progress and outcomes in alignment with facility policies and payer requirements.
- Collaborate with an interdisciplinary team (nursing, therapy assistants, physicians, and social work) to coordinate patient care, discharge planning, and goal achievement.
- Provide direct therapy sessions within the guaranteed weekly hours; modify goals and approaches based on resident response and changing needs.
- Educate residents and families on home exercise programs, safety strategies, and activity adaptations to promote ongoing rehabilitation and fall prevention.
- Ensure compliance with documentation, regulatory standards, and facility policies; participate in quality improvement and performance improvement activities as assigned. Requirements
- Active Occupational Therapist license in Washington state.
- Graduation from an accredited OT program; NBCOT certification or eligibility is preferred.
- Minimum 1 year of clinical OT experience, with preference for acute care or skilled nursing facility exposure.
- Ability to work effectively in a skilled nursing facility environment and to deliver services within a 5x8 day shift.
- Willingness to travel for a 13-week assignment starting 11/23/2026; goodStanding professional behavior and ability to meet assignment requirements. Benefits and Support
