Requirements: Current state license as a Registered Nurse; Homecare Homebase Software Experience - Required; OASIS Experience - Required; Proof of current CPR certification - Required; 1-2 years experience as a Registered Nurse in a clinical care setting - Required; 2+ years Home health experience - Required.

Responsibilities:

• Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.

• Develop, implement, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.

• Educate patients and families regarding care, medications, and safety, utilizing appropriate teaching methods.

• Provide supervision and clinical directions to Home Health Aides and other staff to maintain service quality.

• Coordinate referrals to appropriate agencies as needed and ensure compliance with third-party payer requirements.

• Perform on-call services and communicate effectively with patients, families, and healthcare team members.
