About This Role As a Registered Nurse Case Manager in Palliative Care at Massachusetts General Hospital, you will collaborate with the healthcare team to enhance patient care services and ensure high-quality patient experiences. Highlights Location: Boston, MA Schedule: Every other weekend, 8 hours per week Sign-On Bonus: Up to $25,000 What You'll Do Collaborate with the health care team to enhance patient care services along the continuum of care. Meet patient needs efficiently and expeditiously. Continuously improve the patient experience. Ensure institutional standards of high-quality patient care. Reduce costs and ensure reimbursement. Evaluate, predict, and facilitate the trajectory of patient care. Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation. Requirements Education Graduate of an accredited nursing program. Experience Previous experience in case management preferred.
