Purpose of this Position:This position provides case management services, with a portion of responsibilities dedicated to supporting incarcerated individuals preparing for re-entry into the community following release from the Wyoming County Correctional Facility. The role will also maintain a caseload of members not participating in the re-entry program. Services will primarily focus on the adult population, while also providing support to youth and families as needed. Responsibilities include advocating for consumers and assisting them in navigating mental health services and resources related to Social Determinants of Health. Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,B. Registered nurse; orC. A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years’ experience in direct contact with mental health consumers; orD. A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989. #J-18808-Ljbffr
