What You’ll Do: Provide home health physical therapy services to adult and geriatric patients Perform evaluations and develop individualized care plans Deliver rehabilitation services focused on strength, balance, and mobility Educate patients and caregivers on treatment plans Collaborate with physicians and interdisciplinary teams Complete accurate and timely EMR documentation What We’re Looking For: Licensed Physical Therapist (PT) in DC, Maryland, or Virginia (or eligible) Doctor of Physical Therapy (DPT) required New grads welcome — mentorship available Home health experience preferred, not required Service Areas: Opportunities available across the DMV region , including: Washington, D.C.
