Responsibilities include: Under the supervision of a pharmacist, obtains accurate and complete medication histories for patients admitted to the acute care facility. This position supports clinical pharmacists and providers by obtaining, verifying, and documenting patients' medication histories through direct patient interviews, caregiver communication, and review of external sources such as pharmacy records and prescription databases. The technician contributes to patient safety by minimizing medication discrepancies and supporting optimal pharmacotherapy decisions.
