RN or LPN Part Time or FT 11pm to 7am

Job Summary:

As a key member of our night shift, you will play a vital role in delivering high-quality patient care to our residents. Responsibilities:

• Provide direct patient care to residents, including administering medications, assisting with daily living activities, and monitoring vital signs

• Collaborate with the healthcare team to develop and implement individualized care plans

• Conduct routine vital sign assessments, monitor and report changes in resident condition, and respond to emergencies

• Maintain accurate and up-to-date records of patient care, medications, and treatments

• Participate in incident reporting, quality improvement, and infection control initiatives

• Provide education and support to residents and their families

Requirements:

• Current licensure as an RN or LPN in the state of Illinois

• High school diploma or equivalent required; associate's or bachelor's degree preferred

• 2 years of experience

• Long term care or rehab experience preferred

• Ability to work 11pm to 7am shifts, including rotating weekends and holidays
