Registered Nurse - Telemetry

Job Summary:
Provides expert telemetry nursing care for a designated patient or group of patients, ensuring adherence to established standards and the nursing process. Oversees and guides the activities of nursing staff at various levels while coordinating interdisciplinary care and utilizing critical thinking and professional judgment.

Responsibilities:

• Assess patients by collecting relevant health data and analyzing assessment findings to determine diagnoses and care needs.
• Develop and implement individualized care plans with clear interventions aimed at achieving patient outcomes.
• Coordinate care delivery, promote patient safety, and create a healing environment through effective communication with the healthcare team.
• Evaluate patient progress towards achieving established outcomes and adjust care plans as necessary.
• Educate patients and their families on health conditions, treatment options, and post-discharge care.
• Collaborate with multidisciplinary teams to ensure seamless discharge planning and continuity of care.

Qualifications:

• Hold a current Registered Nurse (RN) license in the practice state.
• Possess CPR/BLS certification from an approved vendor per facility policy.
• Have a minimum of two years recent experience in a relevant clinical setting, preferably in acute care or telemetry.
• Demonstrate proficiency in Epic electronic health record system and the ability to float between Medical-Surgical and Telemetry units.
• Bachelor’s Degree in Nursing (BSN) preferred.
