Field-Based Care Transition & LTSS Specialist
Humana
Humana is seeking a Transition/Custodial Prevention Specialist to work with members in Illinois, creating personalized care plans and coordinating LTSS. You will meet members in their homes or NF facilities, assess goals and barriers, and connect them with community resources to promote independence and better health outcomes. You will collaborate with care teams, navigate Medicaid eligibility, and support caregivers while traveling regionally to serve members in Kane and DeKalb counties and #J-18808-Ljbffr Humana
