Hospice Registered Nurse Case Manager

Ensure all activities align with Elara Caring’s vision and Hospice team leadership Assess hospice patients and families to identify physical, psychosocial, environmental, psychological, social, and spiritual needs Review documentation, clinical records, Interdisciplinary Team reports, after-hours reports, and on-site evaluations Communicate significant findings, problems, and changes in health condition or environment to the Clinical Supervisor, physician, facility, and other care personnel Counsel and instruct patients, facilities, and families in following the Interdisciplinary Plan of Care Teach hospice philosophy and pain and symptom management to staff in alternate care settings Provide appropriate support at time of death and perform bereavement assessments Report to the Clinical Team Manager Travel 50% as required for patient care and job tasks Requirements Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current state license as a Registered Nurse (RN) 1 year of experience in a clinical care setting Experience in a hospice or home health environment preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance Reliable transportation to perform job tasks Core Competencies Demonstrates expertise in patient assessment and interdisciplinary care coordination, with a strong focus on hospice philosophy, pain management, and bereavement support. Holds a valid Registered Nurse license and possesses experience in clinical care settings, particularly in hospice or home health environments.
