Occupational Therapist (OT)

Job Responsibilities Provide home‑based occupational therapy services as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to the physician and relevant agency staff. Develop and adjust individualized OT plans of care based on patient needs. Evaluate patient functioning using diagnostic and prognostic procedures. Create and implement activity and home exercise programs to improve range of motion, muscle strength, and coordination. Teach patients and families preventive measures to reduce re‑hospitalizations. Advise patients and families on disease/injury management and proper use of equipment such as wheelchairs, braces, walkers, and prosthetic/orthotic devices. Oversee and supervise the total care of patients provided by the Certified OT Assistant (COTA). Communicate with the COTA on supervisory visit results and collaborate with agency staff. Maintain all clinical documentation in accordance with protocol and Medicare/Federal guidelines. Attend in‑service training and mandatory agency meetings. Comply with all agency policies, procedures, rules, and regulations. Hard & Soft Skills Compassionate communicator with a positive attitude. Patience when working with patients, families, physicians, and coworkers. Attention to detail and observant follow‑through on directions. Qualifications Current Occupational Therapist license in the state of employment. Minimum one‑year experience as an OT in an acute care setting (home‑care experience preferred).
