Community Health Advocate & Care Navigator

Community Health and Wellness of Greater Torrington, Inc.

Community Health and Wellness Center is seeking a Community Health Worker (CHW) to assist patients and families with navigating community and social services to adopt healthy behaviors. The CHW supports Providers and the Care Management Team through an integrated care approach, offering social support, informal counseling, education and advocacy. Responsibilities include outreach, home visits, health screenings, and referrals. #J-18808-Ljbffr
