Nurse Practitioner, Interventional Pain Management

CenterWell Home Health is seeking a Nurse Practitioner, Interventional Pain Management to deliver advanced in-home care to seniors and adults with chronic pain conditions. In this role, you will perform comprehensive assessments, develop and manage interventional pain treatment plans, prescribe medications, and coordinate care with an interdisciplinary team. You will educate patients and caregivers on pain management strategies, monitor treatment effectiveness, and adjust plans as needed.
