Physician's Assistant

Signify Health is seeking a Physician Assistant to perform in-home health evaluations and support value-based care programs. You will conduct comprehensive assessments, review medical histories, perform focused exams, and document findings in our EMR to inform care plans and risk adjustment. Collaborating with interdisciplinary teams, you’ll use analytics and technology to close care gaps, educate patients, and coordinate follow-up with primary and specialty providers. This role blends autonomous clinical practice, field-based work, and data-driven decision-making to improve outcomes for vulnerable populations while supporting work–life balance in a mission-driven culture.
