Job Details:
Schedule: Monday-Friday, 8:00 AM-4:30 PM EST; On-call one day per week and one weekend per month
Location: Farmington Hills, MI
Coverage Areas: Within the service zone, including Oakland, Wayne, Livingston, Washtenaw, Macomb, and Genesee Counties
Job Type: Local Contract
Contract Length: 13 Weeks
Start Date: ASAP
Weekly Hour Guarantee: 40 hours/week
Pay Range: $52.00-$54.00/Hour
*Pay offered is based on experience, expertise, credentialing, and education.
Mileage: Reimbursed between visits at the applicable mileage rate

Duties:

• Conduct comprehensive in-home patient assessments, develop and update individualized care plans, and monitor patients for changes in condition or complications.
• Provide skilled nursing care, including medication administration, wound care, post-hospital or post-surgical care, IV therapy when ordered, and chronic disease management.
• Educate patients and caregivers on medications, disease management, self-care, infection prevention, and home safety while advocating for patient needs and promoting independence.
• Coordinate care with physicians and interdisciplinary team members while completing accurate, timely clinical documentation and maintaining Medicare, CMS, and agency compliance.

Requirements:

• License(s): Active, Registered Nurse (RN) license
• Certification(s): Basic Life Support (BLS) Certification
• Years of experience:1+
• Skills/Specialties/Technology: Clinical assessment and judgment, medication and treatment management, wound care, post-acute care, effective communication, time management, organization, independent clinical decision-making, HomeCare HomeBase (PointCare) EMR experience, valid driver's license, and reliable transportation
