Description
  • Conduct thorough evaluations for patients in inpatient or outpatient settings and formulate personalized treatment plans.
  • Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
  • Collaborate with a diverse team including nurses, physicians, case managers, and rehabilitation professionals to maximize patient outcomes.
  • Educate patients and their families on therapeutic objectives, discharge plans, and exercises for home.
  • Maintain accurate documentation of evaluations, treatments, and progress in line with facility as well as regulatory standards.

Education
  • Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution.
Certifications
  • State licensure as a Physical Therapist .
  • CPR certification preferred.
