- Conduct thorough evaluations for patients in inpatient or outpatient settings and formulate personalized treatment plans.
- Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
- Collaborate with a diverse team including nurses, physicians, case managers, and rehabilitation professionals to maximize patient outcomes.
- Educate patients and their families on therapeutic objectives, discharge plans, and exercises for home.
- Maintain accurate documentation of evaluations, treatments, and progress in line with facility as well as regulatory standards.
- Engage in interdisciplinary care rounds and meetings to ensure a seamless healthcare approach.
- Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution.
- State licensure as a Physical Therapist .
- CPR certification preferred.
- Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory.
- Prior experience is strongly encouraged.
- Ability to manage patients with intricate medical and mobility complications.
