Document accurately and on time in the EMR (assessments, care provided, orders, signatures) to support billing, compliance, and continuity. Coordinate care & communicate with physicians, therapists, care coordinators, and admin to align orders/authorizations and keep care plans current. Monitor progress & escape concerns (changes in condition, safety risks, non‑compliance) to prevent adverse events and avoid unnecessary hospitalizations. Support compliance & quality - follow policies, payer requirements, and home health standards; participate in QA and care plan updates.
