A few position highlights include:Make assigned patient care visits in accordance with physician orders. Complete supporting documentation accurately and timely, including visit report, OASIS assessment, plan of care and revisions, medication profile, progress notes, recertification, discharge/transfer summary, etc.Attend meetings for all patients assigned. Participate and offer input regarding each patient’s care and progress toward individual patient goals.Understand and adhere to all agency policies, procedures, and processes (administrative, personnel, clinical, etc.).
